Fuel to Heal - The Surprising Power of Exogenous Ketones for Recovery, Focus, and Clean Energy

We tend to think of energy as a lifestyle issue — something we chase with coffee when the afternoon drags. But energy is also a healing issue. Every time your body repairs a torn joint, a bruised brain, or an inflamed tissue, it's running on fuel. And when that fuel supply falters, healing slows down.
I recently attended a lecture by Dr. Elizabeth Yurth, a physician known for her work in cellular medicine and longevity, on the role of exogenous ketones in recovery. What she shared reframed how I think about ketones — not just as a clean energy source (though they're excellent for that), but as a genuine tool for helping the body heal after injury. I want to walk you through what I learned, and then broaden it into why ketones have become one of my favorite recommendations for brain fuel, steady energy, and metabolic flexibility.

First, a quick primer: what are exogenous ketones?
Your body runs on two main fuels: glucose (from carbs and sugar) and ketones. Ketones are made naturally in your liver during fasting, sleep, low-carb eating, or long exercise. The primary one, beta-hydroxybutyrate (BHB), is a compact, efficient molecule your brain and muscles can burn directly.
“Exogenous” simply means from outside the body. Exogenous ketones are a ready-to-use source of BHB you drink — raising your blood ketone levels within about 15–30 minutes, without needing to fast or follow a strict keto diet. Think of it as topping off your fuel tank on demand.
Why ketones matter after an injury
Here's the piece from Dr. Yurth's lecture that stuck with me most.
When you sustain an acute injury — a brain injury, a concussion, a joint injury — the damaged tissue goes into a kind of metabolic crisis. Blood flow drops, oxygen becomes scarce, and the mitochondria (your cells' power plants) get stunned. ATP, the body's energy currency, plummets. That sudden energy failure triggers a damaging cascade: inflammation, oxidative stress, and excitotoxicity.
To make matters worse, glucose metabolism needs oxygen — and in an oxygen-starved, injured tissue, there isn't enough. So the body is forced into inefficient anaerobic glycolysis, which produces lactate and tends to worsen pain and tissue damage.
This is exactly where ketones shine. According to Dr. Yurth, ketones offer several advantages precisely when injured tissue needs them most:
More energy, less oxygen. BHB yields roughly 25% more ATP per molecule of oxygen than glucose. In hypoxic (low-oxygen) injured tissue, that efficiency is critical — ketones keep the cell powered when glucose can't.
Fewer damaging byproducts. Ketone metabolism generates fewer reactive oxygen species than the oxidative phosphorylation glucose relies on, meaning less oxidative stress on already-stressed tissue.
A direct anti-inflammatory signal. BHB inhibits the NLRP3 inflammasome — a master switch at the top of the inflammatory cascade — blocking the assembly of powerful inflammatory cytokines like IL-1β and IL-6 that drive much of the damage.
Gene expression that favors repair. Researchers have observed BHB upregulating protective antioxidant enzymes (like superoxide dismutase) and stimulating PGC-1α, which drives mitochondrial biogenesis — the making of brand-new mitochondria. More mitochondria and more ATP is exactly what a healing body needs.
The brain benefits, specifically
Because ketones cross the blood-brain barrier immediately, they're especially valuable after a brain injury, when glucose metabolism is rapidly impaired and the brain is starved for energy. Dr. Yurth highlighted several brain-specific effects:
Ketones help increase cerebral blood flow rather than let it decrease.
They support GABA, the brain's calming, inhibitory neurotransmitter.
They upregulate BDNF (brain-derived neurotrophic factor), which supports the growth and repair of brain cells.
This is also why she suggested a protective use: if you do any high-risk sport — skiing, mountain biking, football, soccer — consider a can or shot of ketones before you play. Having neuroprotection on board is a meaningful win.
Less pain, faster healing
Because ketones quickly reduce inflammatory mediators and improve mitochondrial function, they also help modulate pain. Start ketones soon after an injury, and your pain burden may be lessened; even in long-term pain situations, people can see gradual improvement as the healing cascade is corrected.
Dr. Yurth described a rough timeline worth understanding: improved ATP, reduced inflammation, and better cerebral blood flow can show up within hours; gene-expression changes and antioxidant upregulation take around 72 hours; and new mitochondria forming can take three months or more. That's why, after an injury, she suggests continuing ketones at a higher dose for at least six weeks — often three to four servings a day during the acute phase. Not all ketones are created equal
One of the most practical parts of the lecture was on delivery form — because it turns out how ketones are packaged matters a lot.
Pure ketone esters bond BHB to 1,3-butanediol. They spike ketones fast but only briefly — typically 30–60 minutes — then drop off rapidly, often with a crash back to baseline. They're famously harsh-tasting (think “rocket fuel”), can cause GI distress, and because the taste can spike cortisol, they've even been shown to raise blood sugar over time. To stay in a high-ketone state for healing, you'd have to re-dose every couple of hours — unrealistic for most people.
Ketone salts come loaded with minerals and electrolytes. That heavy salt load is hard on the GI tract, and you can't dose the ketones high enough without overdoing the salt.

Lean on it too hard and you build tolerance, need more to feel the same lift, and disrupt the sleep that would have fixed the tiredness in the first place — a dependence cycle many of us know well.
